Collaborative Drug Therapy and Disease State Management

Our Board Certified Clinical Pharmacist Practitioners have collaborative practice agreements with physicians in practice (Patient-centered Medical Homes and Accountable Care Organizations). Pharmacist Practitioners collaborates with physicians in order to manage complex polypharmacy and chronic disease states (Asthma, Chronic Pain, COPD, Diabetes, Hypertension, Hyperlipidemia, Hypothyroidism, Healthy weight Loss, Services for Pharmacogenetics and Nutrigenomics, Health and Wellness Care).

Medication Therapy Management by Board Certified Pharmacist include:

 

  • Review all medications including supplements, herbals and other health products.

  • Screen for side effects/adverse effects, unnecessary medications, interactions among medications, supplements, other health products, medical conditions you may have and foods/drinks you might be taking.

  • Ensure that medications are appropriate, there is no overdosing or under dosing of medications.

  • Identify duplication of therapy, unnecessary therapy or additional therapy needed.

  • Establish a treatment goal based on evidence based clinical practice guidelines that is specific for patient.

  • Incorporate a wellness plan into treatment plan.

  • Communicate and coordinate with patient’s medical providers.

  • Implement safe medication use, compliance, adherence and decrease pill burden.

  • Provide a personal medication list and a personal action plan at the end of a session.
